8. While Gore-tex suture has several advantages for use in scleral-sutured IOLs, it must be adequately buried in the sclera to prevent erosion. 

9. 5-0 polypropylene fixation using the Canabrava double-flanged technique is a good option for re-fixating lenses and avoids several of the pitfalls of sutured IOLs. 

10. Re-fixation of a pre-existing lens requires familiarity and comfort with all different approaches to secondary IOLs and complex anterior segment surgery.

Dr. Safran is an American Board Certified, Fellowship-trained Cornea Specialist. His special interest is in cataract surgery with premium lenses. He is extremely knowledgeable regarding a wide variety of eye diseases and is particularly adept at treating patients with delicate or complicated conditions.

Dr. Safran has a Youtube channel, where he shares his knowledge of very complex eye surgery cases! His channel has over 180 videos on complex anterior segment surgery that are viewed by thousands of surgeons and used by many teaching programs for instructional purposes.

The Video Journal of Cataract & Refractive Surgery (JCRS) is a monthly, preeminent, peer-reviewed medical journal covering refractive and lens-based optical surgery. JCRS has maintained its status as a prominent forum for original research, review, and evaluation of refractive and lens-based surgical procedures for more than 30 years.

The JCRS is the official journal of the International Society of Refractive Surgery, a partner of the American Academy of Ophthalmology. The first issue of the 2021 Video Journal of Cataract, Refractive, & Glaucoma Surgery entitled “History and Evolution of Contemporary Refractive Surgery” is available for viewing.

As a team of ophthalmologists, ophthalmology residents, and medical students here at the University of Calgary, we are dedicated to exploring and developing simulation-based learning endeavours in surgical ophthalmology.

In this project, we provide walkthrough videos of the levels of the VR Magic EyeSi® Surgical simulator. A staple in many ophthalmology training programs, the EyeSi® system provides virtual modules to practice surgical skills in cataract surgery. However, there are no specific walkthroughs and tips provided by the simulator in completing each individual level. Hence our library of over 100 videos on Youtube® include individualized advice and narration on video walkthroughs on how we approached each level, and our final scoring as well. We hope these videos help ophthalmology residents in completing all the levels in the EyeSi® cataract simulator, that will translate directly into real-life operating room success!

Target Audience

This free and accessible series of videos has been designed to meet the educational needs of ophthalmology residents learning how to perform cataract surgery and associated complications on the VR Magic EyeSi Simulator®; it may also serve as a teaching resource for reference by ophthalmology faculty. It would also be helpful for medical students who wish to visualize various ophthalmic procedures for their interest and also explore current simulator training.

Learning Objectives

In watching the walkthrough videos, viewers will be able to:

• Understand the goals and objectives for each level in the EyeSi® Cataract Simulator
• Utilize the videos in our series to appreciate the core mechanic and manuevers necessary for scoring well for each level in the EyeSi® Cataract Simulator
